2010-2011 Back-to-School Immunizations

1. Proof of Residency (utility bill showing name and current date, i.e., electric bill,
water bill, gas bill, lease contract, closing contract); If you are building in FISD or
residing with relatives or other persons having a legal residence in FISD, you
must acquire the proper documentation from our administration office to show
proof of residency.
2. Original State Birth Certificate (no hospital certificate)
3. Child’s Social Security Card
4. Most recent grade transcript (report card)
5. Driver’s License of Guardian
6. Immunization Record showing the following:
DPT (Diphtheria, Pertussis, Tetanus): At least three (3) doses. Last dose received
on or after 4th birthday.
POLIO: At least three (3) doses of Polio vaccine (OPV or IPV) are required, provided at
least one dose has been received on or after the 4th birthday.
MMR: Two (2) doses are required. The first dose must have been administered on or
after the first birthday.
HEPATITIS B: Three (3) doses are required.
VARICELLA (Chickenpox): One (1) dose of Varicella (Chickenpox)vaccine on or
after the 1st birthday OR documentation of previously having the illness.
***The day, month, and year of vaccine administration must be recorded on all new
school records.
THE ABOVE IMMUNIZATION REQUIREMENTS MUST BE MET BEFORE A STUDENT WILL BE ALLOWED TO REGISTER FOR SCHOOL.
Bring your child’s medical records with you at the time of registration. If there is a question about
shots, the problem can be addressed at that time.
