If you missed registration day. . .
New Students: If you are a new student to Friendswood ISD, you may call the Counseling Center (281.482.7818, ext 207) to schedule an appointment. A parent or legal guardian must accompany the student at the time of the appointment. Registration information is below.
Bring your child's shot records with you at time of registration.

Residency
Students must be residents of Friendswood ISD in order to attend FISD schools. Proof of residency must be provided to the campus registrar at the time of registration. Proof includes a current utility bill or lease agreement with name of parent / guardian and physical address. Any person who knowingly falsifies information on a form required for a student's enrollment in the District shall be liable to the District for full tuition if the student is not eligible for enrollment. Presenting a false document or record under Section 21.0313 of the Texas Education Code is an offense under Section 37.10, Penal Code. Students found to be residing outside of the District's boundaries will be immediately withdrawn from school

2008- 2009 Registration Information


 The following items are necessary for registration:

  1. Proof of residency (Closing documents, lease agreement [signature of the leasee and property owner], or utility bill)
  2. Previous report card
  3. Student's social security card
  4. Immunization records (MUST have at time of registration)
  5. Most recent grade transcript (if not on withdrawal form)

 Required Documents (originals and copies)

1. Proof of Residency (utility bill showing name and current date, i.e., light bill, water bill, Entex bill, lease contract)
2. Original State Birth Certificate (no hospital certificate)
3. Child's Social Security Card
4. Immunization record

If you missed registration day. . .
On Thursday, August 21 from 1 - 4 pm we will be having a day for students who were unable to register on August 11 & 12 to turn in their packet information and pick up their books and schedules. This is for RETURNING STUDENTS ONLY!!

Texas Immunization Requirements

Texas Law requires specific immunizations for all students enrolled in schools. All required immunizations must be completed, validated by a doctor or health department, and on file at school the first day of classes. The following is the immunization schedule for students in Texas:


DPT (Diphtheria, Tetanus Toxoid, and Pertussis) vaccine:
5 doses unless the 4th dose was given on or after 4th birthday. One dose of Td is required 10 years after the last dose of DTP vaccine.

POLIO: (IPV/OPV) vaccine:
4 doses required unless the 3rd dose was given on or after 4th birthday.

MMR (Measles, Mumps, and Rubella) vaccine: 2 doses with the first dose given on or after the 1st birthday and the 2nd dose given by age 5 or entry into kindergarten.

HEPATITIS B: Three (3) doses are required.

VARICELLA (Chickenpox): One (1) dose of Varicella (Chickenpox) vaccine on or after the 1st birthday 2 doses if vaccine given at 13 years of age or older. Documentation of child having disease meets this requirement also.


During junior high some students may be due for their TD booster. You will be notified by mail over the summer if your child is due an immunization during the next school year. You are required to get this immunization prior to attending classes. Please bring a copy of your immunization to registration or mail a copy to the school -- Attn: School Nurse.
The above immunization requirements must be met BEFORE a F.J.H. STUDENT will be allowed to attend school.

REGISTRATION PROCEDURES

On the day of registration, entry into the school will be located at the side door, nearest the cafeteria. Please enter the building from the teacher parking lot. Registration stations are as follows:

1. Cafeteria ­ Verification Sheet

2. Verify data on the sheet and make needed changes. Receive bar code.

3. Spirit Hall Optional PTO membership, student directory, fundraiser and Spirit Store.

4. Room 4 - Turn in forms: Acknowledgment, Health Data, Emergency Relay, Use of Computer & Directory Information.

5. Room 3: Fee Statement - Fees will be paid here

6. Checkout Textbooks

7. Room 20 Purchase/Pickup School Supplies
