Residency
Students must be residents of Friendswood ISD in order to attend FISD schools. Proof of residency must be provided to the campus registrar at the time of registration. Proof includes a current utility bill or lease agreement with name of parent / guardian and physical address. Any person who knowingly falsifies information on a form required for a student's enrollment in the District shall be liable to the District for full tuition if the student is not eligible for enrollment. Presenting a false document or record under Section 21.0313 of the Texas Education Code is an offense under Section 37.10, Penal Code. Students found to be residing outside of the District's boundaries will be immediately withdrawn from school

Current students who cannot participate in regular registration should show up the first day of school with a parent, prepared to register and pay fees.

New to District students who cannot participate in New to District registration show up the SECOND day of school with a parent and all necessary information, including current shot records and proof of residency, prepared to register and pay fees.

 


                     

2011-2012 REGISTRATION

If you are new to the district, the following documents are required for registration.
ONLY A LEGAL GUARDIAN MAY REGISTER A STUDENT.
WHAT TO BRING TO REGISTER:  (Originals and COPIES):

  1. Proof of Residency (utility bill showing legal guardian name and current date, i.e., light bill, water bill, gas bill, lease contract, closing papers).   If you are building in FISD or residing with relatives or other persons having a legal residence in FISD, you must acquire the proper documentation from our administration office to show proof of residency.
  2. Driver’s license of legal guardian
  3. Original State Birth Certificate (NO hospital certificate)
  4. Child’s Social Security Card
  5. Immunization records
  6. Previous Report Card
  7. Most recent grade transcript (if not on withdrawal form)

In order to best utilize instructional time during the first week of school, we ask that you complete all required paperwork prior to the start of school.  On the days of packet pick-up, you simply pick up a folder with copies of the forms needed for registration.  You fill out these forms at home and bring them with you on the scheduled day of registration listed below.

Texas Immunization Requirements
Texas Law requires specific immunizations for all students enrolled in schools. All required immunizations must be completed, validated by a doctor or health department, and on file at school the first day of classes. The following is the immunization schedule for students in Texas:
DPT (Diphtheria, Tetanus Toxoid, and Pertussis) vaccine:
Beginning August 1, 2009, all students entering the 7th Graded are required to have a booster dose of DPT only if it has been 5 years since their last dose. Students in grades 9-12 are required to have a booster dose of DPT if it has been 10 years since their previous dose.
POLIO: (IPV/OPV) vaccine:
4 doses required unless the 3rd dose was given on or after 4th birthday.
MENINGOCOCCAL Vaccine: Beginning August 1, 2009, all students entering 7th Grade are required to have one dose of Meningococcal vaccine.
MMR (Measles, Mumps, and Rubella) vaccine: 2 doses with the first dose given on or after the 1st birthday and the 2nd dose given by age 5 or entry into kindergarten.
HEPATITIS A: Beginning August 1, 2009, all students entering Kindergarten are required to have two doses of Hepatitis A vaccine.
HEPATITIS B: Three (3) doses are required.
VARICELLA (Chickenpox): Beginning August 1, 2009, all students entering Kindergarten and 7th Grade are required to have two doses of Varicella vaccine. Documentation of child having disease meets this requirement also.
During junior high some students may be due for their TD booster. You will be notified by mail over the summer if your child is due an immunization during the next school year. You are required to get this immunization prior to attending classes. Please bring a copy of your immunization to registration or mail a copy to the school -- Attn: School Nurse.
The above immunization requirements must be met BEFORE a FJH STUDENT will be allowed to attend school.
