2010-2011 Back-to-School Immunizations
Early Childhood (Students must be 4 on or before September 1, 2010) Children whose parent(s) is/are active in the military, or who qualify as economically disadvantaged (free & reduced lunch) are eligible to enroll for our preschool program, as well as students who are ELL (English Language Learners), those who are homeless, or those who have been in foster care. Proof of the “qualifier” (military - ID, economically disadvantaged –foodstamp, paycheck, ELL – tested, foster care – letter) will be required. Early registration begins Thursday, May 6, 5:00 – 6:30 p.m. in the Westwood Cafeteria
Kindergarten Round-up
(Students must be 5 years old by September 1, 2010)
Special registration for students who will be Kindergarten students next year will be Thursday, May 6, 5:00 – 6:30 p.m. in the Westwood Cafeteria.
Open Registration for all NEW ECP-3rd grade Students
Beginning Monday, May 10, you may register between 8:00 - 12:00 on the following dates:
May 10-14, 17-21 and June 1-3. Registration will resume August 3, 2010.
The following Documents are required for registration:
* Original Birth Certificate (NO hospital certificate)
* Child’s Social Security Card
* Previous Report Card (does not apply to Kindergarten or ECP students)
* Driver License of Parents/Guardians
* Immunization Records
* Proof of Residency - utility bill (showing legal guardian name, service address and current date, either light bill, water bill, or gas bill) lease contract, closing papers. If residing with relatives or other persons having a legal residence in FISD you must have paperwork notarized.
* If you are building in FISD, you must acquire the proper documentation from our administration office to show proof of residency. |
