REGISTRATIONS, ADMISSION, TRANSFERS

Bring your child's medical records with you at time of registration. If there is a question about shots, the problem can be addressed at that time. 
FISD does not accept transfer students who do not reside within district boundaries unless the student becomes a non-resident during the course of the semester or during their 11th grade year. Families who have an executed contract to build or purchase a residence in FISD may attend on a tuition basis. Refer to FISD Board Policy FDA Local for more details.

ONLY A LEGAL GUARDIAN MAY REGISTER A STUDENT.
WHAT TO BRING TO REGISTER:  (Originals and COPIES):

  1. Proof of Residency (utility bill showing legal guardian name and current date, i.e., light bill, water bill, gas bill, lease contract, closing papers).   If you are building in  FISD or residing with relatives or other persons having a legal residence in FISD, you must acquire the proper documentation from our administration office to show proof of residency.

  2. Driver’s license of legal guardian

  3. Original state birth certificate (NO hospital certificate)

  4. Child’s Social Security card

  5. Immunization record showing the following:

DTP/DTaP or DT - 4 doses of DTP/DTaP or DT
Oral Polio - 3 doses of vaccine
MRR - 1 dose of vaccine; given after 1st birthday
Hepatitis B: - 3 doses of vaccine
Varicella (Chicken Pox) - 1 dose requirement for entry into ECP; given after 1st birthday
Hepatitis A - 2 doses requirement for entry into ECP; given after 1st birthday
            HIB - 3 doses

OPEN REGISTRATION WILL BE:
AUGUST 10-14, 2009  MONDAY – WEDNESDAY  8:00 – 12:00
